Information we collect through this website
When you contact us through the website, we may collect the information you choose to provide, such as your name, email address, telephone number, preferred location, and message. We may also collect limited technical information needed to operate and secure the website, such as device, browser, IP address, pages visited, referring page, and event logs.
Please do not submit medical records, diagnoses, insurance numbers, payment-card information, Social Security numbers, or other highly sensitive information through a general website inquiry. If you need to share medical information, please use the method our team provides for that purpose or contact the office directly.
How we use website information
We use website information to respond to requests, help coordinate the next appropriate step, maintain website security, prevent abuse, improve publicly available website content, and meet applicable legal obligations. We do not sell consultation-request information or patient health information to third parties for their independent marketing.
Advertising and measurement
We may use analytics or advertising-measurement services to understand whether our public advertising and website content are effective. If those services are enabled, we will use them only for permitted business purposes, provide notice and obtain consent where law requires it, and update this notice when a material change is made.
We do not use the content of an inquiry, appointment details, diagnoses, treatment information, or other patient health information to create advertising audiences. We do not intentionally place personally identifiable information in website URLs or disclose it to advertising platforms through remarketing or conversion tags. You can control many cookies through your browser settings; where a consent choice is required, we will provide it before optional advertising or analytics technologies operate.
When information may be shared
We may share information with service providers that help us operate the website or communicate with you, but only as needed for those services and subject to appropriate privacy and security obligations. We may also disclose information when required by law, to protect rights and safety, or in connection with a business transition as permitted by law. We do not share mobile-text-message opt-in data or consent with third parties for their own marketing or promotional purposes.
Security and retention
We use reasonable administrative, technical, and physical safeguards designed to protect information. No internet transmission or storage system is completely secure. We retain website information only for as long as reasonably necessary for the purposes described here, to comply with law, resolve disputes, and maintain business records.
